What We Offer our Home Health Nurses:

· Pay Range: $38.91 to $48.63/hour, commensurate with experience

· We reimburse mileage at $.67/mile - This is non-taxable!!

Status: Non-exempt/hourly , Full or Part-time

Area of Service: Vail/Eagle Valley

Essential Functions of the Job:

• The Home Health Nurse performs initial assessment upon admission to Home Health
• Admits patients to Home Health service by accurate entry of patient information, admitting diagnosis and physician's orders
• Develops and implements an individualized, realistic, goal and outcome-focused Plan of Care for each patient and updates the Plan of Care per department guidelines. Plan of care relates to admitting diagnosis and/or to changing needs of patient
• Case manages patient care when assigned to perform Case Management Duties
• Home Health Nurses must participate in Call rotation with one call shift per week and one weekend of call per month

Qualifiications for our Home Health Nurse

• The Home Health Nurse must possesses current license to practice as a registered professional nurse in the state of Colorado.
• Current BLS verification.
• Minimum one year of Home Care nursing experience
• One year of acute care experience
• Personal vehicle for on-the-job transportation in all road and weather conditions, climate

Compensation details: 84000-102000 Yearly Salary
